By checking this box, I understand that my child is auditioning for the Northwest Florida Ballet 2025-26 Trainee, Company, and Pre-Professional Program. I hereby hold harmless and covenant not to prosecute Northwest Florida Ballet or the instructors in the case of injury suffered by my family members or me while on the premises. I hereby grant Northwest Florida Ballet the right to use videotapes and other technology to evaluate my child during the 2025-26 NFB auditions. BY MY SIGNATURE BELOW, I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTAND THE TIME AND FINANCIAL COMMITMENTS REQUIRED UPON ACCEPTANCE TO THE TRAINEE, COMPANY, AND PRE-PROFESSIONAL PROGRAM.
